Job summary
As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
- Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
- Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed
