    Home Health/Hospice Nurse Position

    We are seeking a compassionate and dedicated Home Health/Hospice Nurse for a full-time role. Our esteemed client values high-quality patient care and offers competitive compensation and comprehensive benefits.

    Job Responsibilities:
    • Receive patient assignments from the Operations Leader or Clinical Resource Team Leader.
    • Maintain effective communication regarding patient care, business operations, and a positive work environment.
    • Assume responsibility for patient and family care through assessment, planning, implementation, and evaluation.
    • Collaborate with attending physicians, hospice staff, and agencies to optimize patient care and resource utilization.
    • Review care strategies with the Operations Leader or designated personnel.
    • Communicate consistently with attending physicians about patient and family care.
    • Collect and integrate data on physical, psychological, social, and spiritual factors influencing patient and family health into the care plan.
    Qualifications:
    • A minimum of two years of nursing experience, preferably with at least one year in home care.
    • Current BLS Certification.
    • Valid Registered Nurse licensure in South Carolina.
    • Valid driver's license and proof of vehicle insurance.
