    Description

    Summary

    This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

    Duties

    -Establishes and maintains effective working relationships and communicates with Veterans, staff, and other parties as appropriate.
  • Provides case management with members of the outpatient interdisciplinary team and home and community based services is provided in order to coordinate a collaborative effort to meet the agreed upon goals for a Veteran's treatment needs and include liaison with community professionals regarding needed services for Veterans and/or their families.
  • Manages referrals, typically originate from members of the interdisciplinary team, from Veterans and/or their families coming as walk-ins to seek assistance, and from community professionals.
  • -Utilizes knowledge of community resources, makes referrals to the community and other government agencies as appropriate, and coordinates services.
  • Coordinates referrals for services.
  • Facilitates action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner.
  • -Independently conducts psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.
  • Provides services in a remote setting geographically distanced from the main campus without access to social work supervision on site and is accountable for clinical program effectiveness and modification of service patterns.
  • Uses advanced level of skill in assessing and treating the complicated psychosocial problems of Veterans.
  • -Provides consultation, education, and advocacy.
  • Provides consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives.
  • Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment.
  • -Utilizes a range of specialized interventions and treatment modalities in treatment or with specialized Veteran populations, including individual, group, and/or family counseling or psychotherapy, and advanced level psychosocial and/or case management.
  • Possesses advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness.
  • Incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.

  • Acts as the sole mental health provider at the rural clinic location.
  • Possesses knowledge and ability to independently implement treatment modalities, provide educational classes, and/or supportive groups for Veterans and families.
  • -Has knowledge in developing and implementing methods for measuring effectiveness of social work practice and service in PACT, utilizes outcome evaluations to improve treatment services, and to design system changes.
  • Possesses advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services.
  • Coordinates the delivery of specialized psychosocial services and programs.
  • -Provides specialized consultation to colleagues and students on the psychosocial treatment of Veteran in the PACT, as well as role models effective social work practice skills.
  • Provides subject matter consultation to colleagues and students on the psychosocial treatment of Veterans treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
  • -Expands clinical knowledge in the social work profession, writes policies, procedures, and/or practice guidelines pertaining to the PACT Clinical area.
  • Expands clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs.
  • Writes policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.
