- dangerous or impulsive behaviors causing risk for injury, neglect, self-harm, or victimization
- suicidal or homicidal thinking, posing an imminent danger to themselves or others
- in need of emergency medication intervention for stabilization
- having severe difficulty functioning in daily life
- in need of monitoring 24-hours a day in a safe environment
- Plan, implement, monitor, and evaluate actions required to meet the patient's health and human services needs
- Facilitate multidisciplinary communication to maintain ongoing, positive relationships with healthcare team, patients, families, payers/TPA, and community providers
- Facilitate and support team conferences to achieve consensus regarding continuing care plans
- Document interventions, patient progress, education, and referrals in Care Management progress notes in a timely, accurate manner, as per policy
- Initiate discharge planning upon patient admission to hospital
- Ensure patients or parent/guardians are included in and informed of their plan or care and discharge plan. Orient and train family members and provide resources
- Coordinate discharge and post-discharge requirements including referrals to continuing care. Obtain consent from patients for continuing care providers, initiate timely transportation arrangements
- Establishes a rapport with the patient and family to learn what is the most important to them in their hospital experience and their preferences. Communicates findings to the appropriate healthcare team members
- Works with individuals or departments to limit or resolve delays in service and communicates delays to patients and families
- Assesses patient or family questions and concerns and develops a plan to address needs. Communicates information to appropriate person(s) in appropriate time frames to meet patient or family questions, concerns, and/or expectations (Validation through feedback from supervisors, peers, personal knowledge, or family interview regarding the work performance)
- High School Degree and two years of experience in mental health in a hospital setting
- Bachelor's degree in human services related field preferred
- Basic to intermediate computer skills required
- Must be organized and assertive
- Ability to work with all ages
- Vast knowledge and understanding of Electronic Health Records
- Effective verbal and written communication skills
- Ability to work in a fast paced environment
- Ability to work independently and to carry out assignments to completion within parameters of instruction given
