Roles & Responsibilities

Assess patients' mobility, strength, balance and functional status to create individualized care plans
Design and implement therapeutic exercise programs to improve independence and quality of life
Collaborate with nursing, occupational therapy and rehab aides to coordinate patient-centered care
Document patient progress clearly and timely, adjusting plans as patients improve or needs change
Provide education and training to patients and families on safe mobility, fall prevention and home exercise
Participate in interdisciplinary team meetings to advocate for patient goals and discharge planning

Qualifications

Current California Physical Therapist license (required)
Active CPR/BLS certification
Experience working in skilled nursing or post-acute settings preferred
Travel/contract therapy experience is a plus but not required
Strong clinical reasoning and excellent communication skills
Ability to manage a caseload and prioritize patient needs effectively

Education

Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy or equivalent considered

Knowledge & Skills

Skilled in therapeutic exercise, manual therapy techniques and mobility training
Knowledge of fall prevention strategies and adaptive equipment recommendations
Familiarity with documentation standards for skilled nursing facilities
Comfortable working as part of an interdisciplinary rehab team
Good time management, organization and patient education abilities
