    Job Description: Hospice Registered Nurse
    This role plays a critical part in presenting hospice services at the point of admission, initiating individualized plans of care, and coordinating closely with related disciplines to ensure professional service delivery. Key responsibilities include:
    • Conducting admission nursing assessments to identify patient/caregiver needs
    • Developing plans of care for patients/caregivers, incorporating therapeutic, preventive, and palliative nursing actions
    • Coordinating multidisciplinary care and maintaining effective communication with healthcare providers
    • Participating in admission interdisciplinary group conferences and collaborating with assigned Medical Social Workers to address psychosocial needs
    • Maintaining accurate and timely clinical documentation according to VIA Health Partners guidelines
    • Initiating Nursing Assistant care plans and ensuring productive working relationships with interdisciplinary teams and interdepartmental staff

    Requirements and Qualifications
    To succeed in this role, candidates must meet the following criteria:
    • A Diploma or Associates Degree in nursing required; Bachelors Degree preferred
    • Current, active, and unrestricted NC/SC license as Registered Nurse (RN)
    • Current CPR certification; CHPN preferred
    • Minimum 2 years related experience, including direct care of patients with life-limiting illnesses and experience in hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, or medical surgical settings

    The estimated annual salary for this position is $85,000-$110,000, depending on qualifications and location.
