- Assess patients' condition during every visit and chart observations in electronic medical record
- Perform evaluation tasks, including vital signs, data collection with medical history intake, and medication reconciliation, reporting findings to the patient's provider
- Assists outreach team engaging patients in the community
- Coordinate with multidisciplinary teams necessary Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan
- Provide palliative care as needed to keep the patient comfortable
- Make recommendations for devices or tools that might improve the patient's quality of life
- Actively listen to the patient and respond to concerns or requests
- Ability to travel to multiple patients' homes per day
- Perform venipuncture & finger sticks to obtain blood samples.
- Prepare specimens & specimens for lab analysis.
- Educate patients on general health & basic self-care procedures.
- Review, explain & clarify physician orders with patient
- Provide education and instruct patient in use of home equipment
- Actively participates in panel management bi-weekly clinical rounds
- Patient care may be performed in any setting, including, but not limited to, shelter, transitional housing, assisted care and living facility, the patient's home or mutually designated location with privacy.
- The direction provided by a registered nurse, nurse practitioner, or physician to the LVN is (must/will be) available by remotely linking/connecting and at least by telephone
- Assist outreach as needed, locating patients, providing explanation of services, helping to enroll difficult to reach patients.

- Candidates must have certification of accredited school and current Virginia license
