- Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
- Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
- Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter, and Housing Assistance Programs.

- Perform client interviews, intakes and maintain required paperwork/data in agency data tracking system.
- Assist staff with caseloads, helping households in locating safe & affordable housing and tracking client progress in the agency database.
- Run reports in the data system and perform quality control measures in a timely manner, complete data entry, statistical review, and routine paperwork
- Receive referrals from community queue and verify household meets criteria for program.
- Stay informed of program changes and new developments in the community.
- Offer individuals and families emotional support, information about community resources and services, including employment, housing, shelter services, health care, etc., and assist in completing paperwork such as social services applications, legal filings, etc.
- Ability to demonstrate active listening skills, conflict resolution, internal motivation, and flexibility.
- Be familiar with current housing market information, housing trends, available affordable housing resources and develop positive relationships with landlords and property management companies to help assist staff in locating units.
- Conduct ongoing, individualized case-management to individuals/families in program.
- Develop and facilitate support and informational classes/sessions, life skills, and/or group activities designed to help clients explore and resolve self-care issues that may be influencing the clients' progress towards meeting their established goals.
- Maintain accurate and confidential case files.
- Develop relationships within CAPWN, landlords and property management companies.
- Attend and participate in staff meetings and trainings as requested.

- Two years of documented work experience in a related field
- Experience with dual-focus shelter programs homeless shelters, substance abuse programs, social services programs preferred.
- Knowledge of "Housing First" approach to end homelessness preferred.
- Knowledge or understanding of fair housing, tenant/landlord rights and responsibilities preferred.
- Ability to communicate effectively both orally and in writing and good organizational skills
- Must have a valid driver's license with good driving record

- AA Degree in a related field and at least 1 year experience; or a minimum of 3 successful years' experience in a similar position; or a BA degree in a related field

Supportive Services Case Manager

Job Title: Supportive Services Case Manager
Reports To: Housing First Manager
Areas of Responsibility: Under the supervision of the Housing First Manager, this position is responsible for the implementation and delivery of supportive comprehensive case management and advocacy services for individuals and families participating in CAPWN Programs. This position provides services based on the Housing First Model of care with motivational interviewing, strengths-based goal setting, and trauma-informed care approach to work collaboratively with CAPWN Staff to assist with individuals and families they are working with to help stabilize general goals of maintaining housing, increasing family stability, in providing family functioning and increasing the self-sufficiency in the program. This position will help with data entry and quality control over CAPWN programs. This may include running reports as well as checking and updating data.
