    Overview:


    Florida Assertive Community Treatment (FACT) is a dynamic multidisciplinary treatment team dedicated to delivering intensive community-based support to the severely mentally ill adult population.

    The purpose of FACT Care Manager is to be an integral part of multidisciplinary treatment team and carry out the rehabilitation and support functions under clinical supervision to help clients meet their needs in all aspects of living in the community.

    The Case Manager is responsible for completing thorough assessments to develop specific treatment plans that address the individuals needs and challenges to optimize the functioning of recipients by coordinating the provision of quality treatment and support services in the most efficient and effective manner.

    Services and service frequency should accurately reflect the individual needs, goals, and ability of each recipient.


    Rate of Pay:
    $20.00-$22.00/hr.0-2Y = $20.00 per hour3-5Y = $21.00 per hour6+Y = $22.


    00 per hourShift:

    Hours will vary on a rotation basis which will include some weekends, evenings, and on-call coverageEducation and Experience:Qualifications:Bachelor's degree from accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, health science, or a related human services field and One (1) year of full-time experience working with severely and persistently mentally ill adultsORBachelor's degree from an accredited university or college with other majorand Three (3) years of full-time experience working with severely and persistently mentally ill adults.

    Casemanagement experience is highly preferred.

    To be considered for employment, you will be required to undergo a criminal background check, employment references check, and drug screening.

    In addition you must have a c urrent and valid driver's license with no more than 6 points.
