Job Overview
We are seeking a compassionate and dedicated Female Certified Home Health Aide (CHHA) to join our team in San Jose. In this vital role, you will provide personalized, high-quality care to clients in their homes, ensuring their comfort, safety, and well-being. Your expertise will support individuals with diverse needs, including those requiring Alzheimer's and dementia care, hospice support, and assistance with activities of daily living (ADLs). Duties
- Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and catheter care techniques.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe home environment.
- Document all patient interactions thoroughly in medical records while adhering to privacy standards and home & community care protocols.
Requirements
- Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), with current CPR certification.
- Proven experience in home health care or long-term care settings; nursing home experience is highly valued.
- Experience working with individuals who have Alzheimer's disease, dementia, or other cognitive impairments.
- Ability to operate EMR/EHR systems efficiently for documentation purposes.
- Compassionate attitude with excellent patient observation skills and the ability to work independently.
- Physical stamina to assist with mobility tasks using Hoyer lifts or other assistive devices; capable of performing light cleaning and meal prep duties.
- Knowledge of HIPAA regulations to ensure client privacy at all times.
