We are seeking a highly motivated and experienced Registered Nurse Director of Case Management to join our healthcare organization.
The Director of Case Management plays a critical role in ensuring the coordination and delivery of comprehensive patient care services.
This leadership position requires a skilled and compassionate nurse who can manage a team of case managers and drive the development of case management programs that enhance patient outcomes while optimizing resource utilization.
Key Responsibilities:
Leadership and Team Management:
Provide strong leadership to the case management team, ensuring a collaborative and cohesive work environment.
Recruit, train, and evaluate case management staff to ensure a high-performing team.
Set clear goals and expectations for case managers, fostering their professional development.
Foster a culture of continuous improvement and quality assurance within the department.
Program Development and Implementation:
Develop and implement case management programs to optimize patient care and resource allocation.
Collaborate with other healthcare professionals to establish and maintain standards and best practices.
Analyze data to identify opportunities for process improvement and program enhancement.
Patient Assessment and Care Planning:
Oversee the assessment of patient needs and the development of comprehensive care plans.
Ensure that case managers effectively advocate for patient needs and collaborate with healthcare teams to achieve positive outcomes.
Monitor and evaluate care plans for their effectiveness and adjust as needed.
Resource Utilization:
Monitor resource utilization, ensuring cost-effective care while maintaining high-quality standards.
Oversee the utilization review process to optimize utilization of services.
Collaborate with finance and administration to manage departmental budgets effectively.
Compliance and Regulations:
Stay current with relevant healthcare regulations, accreditation standards, and case management guidelines.
Ensure compliance with all applicable laws and regulations, including CMS guidelines.
Develop and implement policies and procedures that align with regulatory requirements.
Interdepartmental Collaboration:
Foster positive relationships and communication with other departments and healthcare professionals to ensure seamless care transitions.
Collaborate with physicians, nurses, social workers, and other healthcare professionals to facilitate efficient patient care.
Quality Improvement and Reporting:
Establish performance metrics and key performance indicators (KPIs) to measure the effectiveness of case management programs.
Analyze data to identify trends, develop action plans, and report outcomes to senior management.
Implement strategies to continuously improve the quality of care and patient outcomes.
Qualifications:
Current and valid Registered Nurse (RN) license.
Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
Certification in Case Management (CCM or ACM) strongly preferred.
A minimum of 5 years of experience in healthcare, with at least 2 years in a case management leadership role.
Proficiency in healthcare informatics and electronic health records (EHR) systems.Strong leadership, communication, and interpersonal skills.
Excellent problem-solving and critical thinking abilities.
