    Summary

    Working under the direction of the Site Director, the Housing Navigator works to provide case management and support services to members receiving housing assistance with the RBHA (AzCH). As part of the Local Coalition to End Homelessness (LCEH), the Housing Navigator of HOPE works in collaboration with RBHA staff to provide supported housing services to members, including screening for eligibility for housing, submitting applications through the Homeless Management Information System (HMIS) and more. The Housing Navigator promotes a recovery based approach to member care that emphasizes hope, respect, self-direction and empowerment by providing peer support, personal assistance, education on community resources and other supportive services, to aid in members' recovery from mental illness and/or addiction disorders.

    Essential Duties

    •Identifies individuals at risk of or experiencing homelessness in their community and service area.

    •Engage individuals and complete risk assessment to assess for vulnerability and enter qualified individuals onto the By-Name-List (BNL) in the HMIS portal.

    •Provide case management and supportive services to members to ensure they remain actively engaged and receive support needed to maintain their recovery in the community. This includes diversion services.

    •Attend all local LCEH meetings, Case Conferencing Meetings, and Outreach meetings and activities; remain engaged with community and grant partners.

    •Coordinate outreach efforts to ensure all individuals in their service area experiencing homelessness are accounted for and receive a homeless vulnerability assessment which is entered and tracked in the EHR.

    •Promotes HOPE, Inc. as the premier peer support organization to the behavioral health community and prospective funding sources.

    •Ensure compliance with and support Executive directives, Agency contracts and established policies and procedures.

    •Provides peer support to members and meets the person where they are at.

    Requirements

    Qualifications
    • High School/GED
    • Graduate from the Recovery Support Institute, required within one year of employment
    • Strong leadership and organizational skills
    • Effective oral and written communications skills
    • Ability to implement organization-wide activities (setting objectives, developing strategies, developing policies and procedures for programming, and organizing the functions necessary to accomplish the activities)
    • Fingerprint clearance required within 90 days of hire.
    Additional Requirements:
    • (Required) Peer - Individuals must have first- hand knowledge of a behavioral health condition or substance abuse issue, whether or not they received treatment or medication or identify as a family member of someone in recovery.
    • Must be at least 21 years of age per licensing requirements.
    • Valid Arizona Driver's license, Driver's license is an essential requirement for performing duties for this position with a Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
    • 1st Aid, CPR and proof of TB within last year
