- Collaborate with a diverse interdisciplinary team, including Nurses, Social Workers, CNAs, and Clinical Assistants.
- Conduct patient and family assessments.
- Offer bereavement support and develop personalized care plans.
- Lead community grief groups and memorials.
- Participate in internal work groups, meetings, and committees.
- Provide counseling support to interdisciplinary teams and community partners.
- Master's Degree in Social Work, Counseling, Divinity, Theology, or equivalent.
- Experience in counseling and spiritual care.
- Knowledge of grief counseling and death-related issues.
- Proficiency in Microsoft Office Suite.
- Strong communication skills, both verbal and written.
- Flexible and able to adapt to organizational needs.
- Commitment to confidentiality and patient privacy.
- Valid driver's license and access to a reliable vehicle.
