Job Summary
The Young Adult Peer Advocate will provide outreach and engagement services to unhoused young adults in Midtown, Lower Manhattan & Brooklyn.
Sustained outreach and engagement attempt for all individuals who are referred,
even if they initially decline services.

- Sustained outreach and engagement attempt for all individuals who are referred,
even if they initially decline services. - Develop meaningful partnerships with community stakeholders providers including Youth Adult Peer Programs Safe Horizon LGBTQIA+ community programs local hospitals NYPD Department of Youth and Community Development Shelter operators housing providers etc.
- Hold individual one-on-one peer support sessions focusing on engagement bridging transition support help increase participant's strengths navigate systems work hospital discharge planning meetings identify best community resources returning patients facilitate crisis interventions referrals hospitalizations as appropriate attend participate team meetings supervisory sessions perform related duties assigned.
