The hospital provides inpatient beds and a comprehensive array of services, including cancer, orthopedic, spine, cardiovascular, neuroscience, advanced digestive care, disease management and prevention, wound healing, outpatient imaging, intensive and progressive care, pain clinic, same-day surgery, emergency department, and community outreach and education programs. Duke University Health System (DUHS) achieved Magnet designation for the third time with DUHS as only one of 17 Magnet-designated healthcare systems in the U.S. and part of the 9% of hospitals that achieve this recognition. U.S. News & World Report ranked Duke Raleigh Hospital as high-performing in orthopedics and five adult procedures/conditions: chronic obstructive pulmonary disease (COPD), heart failure, kidney failure, lung cancer surgery, and stroke.

    Nurse Program Manager / Patient Safety Officer

    General Description of the Job Class

    Independently provide leadership and direction for the Duke Raleigh patient safety program. Responsible for the development of standards and metrics to measure and monitor patient safety performance.

    Retroactively monitor the safety reporting system for reports of hospital process safety events and opportunities. Consult with hospital leadership, nursing leadership, risk management, and others to develop appropriate hospital responses. Lead efforts to determine safety event root causes including root cause analyses, learning from defects, and other established tools.

    Proactively monitor for, investigate, and address potential patient safety risks utilizing all available data sources. Serve as an effective clinical and operational liaison concerning the facilitation and implementation of innovative systems that eliminate potential patient harm. Introduce patient safety scorecards and/or dashboard indicators in conjunction with medical and nursing staff as a mechanism of tracking and adherence to agreed-upon patient safety goals.

    Lead and coach hospital teams to respond to prior or potential safety events in their domain, monitor their efforts, and follow up to ensure intended outcomes. Serve as a professional role model and mentor to staff by working to support the change process. Develop essential collaborative relationships across hospital teams, keeping them informed of patient safety priorities, initiatives, and incidents to maintain patient safety as a high priority throughout the hospital.

    Actively consult with the Duke Health System patient safety team to ensure a standardized and aligned approach to patient safety monitoring, investigation, and response.

    Independently participate in the initial assessment of new hospital units and initiatives, make realistic recommendations based on the findings, and then facilitate the interventions and resources specific to the findings. This approach will serve to align existing safety and quality efforts with organizational priorities.

    Function in a self-reliant manner, positively representing Duke Raleigh Hospital in meetings and discussions across the health system.

    The PSO reports to the Duke Raleigh Hospital Chief Medical Officer.

    Job Responsibilities:

    Planning:

    · Commence and oversee the development of a comprehensive patient safety improvement program inclusive of analysis and trending of data related to hospital initiatives

    · Lead patient safety assessments

    · Develop or help develop action plans according to assessments

    · Remain fully informed of national and local patient safety mandates

    Operations Management.

    · Design and implement effective patient safety and clinical quality programs that will enhance the delivery of effective patient care, maximize patient safety, and minimize patient risk

    · Integrate safety/quality initiatives with hospital operations and administration

    · Engage staff and physicians in program participation

    · Develop and implement policies and procedures that support patient safety and clinical quality initiatives at Duke Raleigh Hospital

    · Analyze processes and opportunities to determine potential enhancements or improvements to facility procedures and activities. Recommend appropriate actions or solutions consistent with overall goals

    · Promote shared accountability initiatives

    · Provide support and advice regarding best practices for patient safety

    · Ensure appropriate targets are developed and met relative to the quality of care.

    · Promote safety standards in accordance with federal and state agencies and The Joint Commission (TJC) standards including National Patient Safety Goals

    · Recommend and facilitate change within the organization to improve patient safety based on demonstrated risks and proactive risk assessment

    · Develop a comprehensive patient safety improvement program inclusive of the analysis and trending of data related to hospital initiatives.

    · Assist in the coordination and prioritization of patient safety and clinical quality initiatives in alignment with hospital priorities

    · Work with all services to hardwire reliable delivery of healthcare systems that incorporate best practice patient safety methodologies.

    · Foster and maintain collaborative relationships with all employees and medical staff in implementing best practices in patient and hospital safety

    · Provide leadership and/or participate in patient safety committees and be responsible for communicating vertically and horizontally within the organization.

    · Help inform and guide teams about accreditation, regulatory, and certification standards

    · Support data surveillance to evaluate safety initiatives and hospital programs' clinical safety.

    Personnel Management.

    · Provide performance feedback of key personnel associated with safety programs

    · Develop and foster effective collaboration between clinical and operational departments to ensure an integrated approach to providing quality and patient safety

    Research and Education Management.

    · Proactively communicate with and educate local leadership and staff regarding patient safety initiatives and activities

    · Compile and prepare reports and analyses setting forth progress, adverse trends, and appropriate recommendations and/or conclusions related to patient care

    · Facilitate the proliferation of clinical research activities related to Patient Safety

    Communication

    · In order to fulfill these responsibilities, the patient safety officer should develop strong working relationships and maintain open communication with all hospital leaders, hospital and system risk management, hospital and system nursing leaders, hospital and system quality leaders, fellow health system patient safety officers, and system patient safety leadership. Developing an effective collaborative relationship is essential to maintaining patient safety as a high priority.

    · The patient safety officer will keep the hospital executive leadership team, system patient safety team, and risk management informed of patient safety priorities, initiatives, and incidents.

    Degrees, Licensure, and/or Certification

    Current or compact RN licensure in the state of North Carolina is required. BLS required.
