    Job Goal: To develop resources, train and coach staff, support transition, and deliver services to students with disabilities with a focus on job exploration, work-based learning, post-secondary education preparation, workplace readiness, and self-advocacy.

    Job Qualifications:
    1. Must have a minimum of a Bachelor's Degree in Special Education, Master's Degree preferred.
    2. Current certification as a Special Education Teacher of Students with Disabilities.
    3. Must have at least 3 years' experience as a Special Education teacher at the secondary level, working with transition planning, activities, and services.
    4. Must have a high degree of organizational skills as well as computer literacy.
    5. Must have excellent oral and written communication skills.
    6. Outstanding ability to motivate and build relationships with students and parents to promote student achievement and strengthen community partnerships.
    7. Experience in training, mentoring, and providing professional development to educators.
    8. Commendable history of effective leadership, extra-curricular participation, an acceptable attendance record, and exemplary performance.
    9. Ability to work collaboratively with diverse stakeholders, including teachers, administrators, industry partners, and community members.
    Essential Functions:
    1. Collaborate with other colleagues, member district administrators and representatives of outside agencies.
    2. Provide information and training to secondary special education teachers.
    3. Support transition compliance (Indicator 13) and maintain current understanding of state and federal transition requirements.
    4. Assist with alignment of the transition continuum of services at the secondary level.
    5. Advocate needed changes in school or district transition programs.
    6. Assist in the development of the IEP.
    7. Facilitates referral, application, eligibility process for student and families to adult service providers.
    8. Participate in projects and committees as requested by the district office personnel.
    9. Establish networks of support for families and students.
    10. Create informational documents for parents regarding transition, vocational opportunities, and waivers.
    11. Organize presentations and / or transition fair to provide families information regarding access to adult providers and services.
    12. Coordinate with staff to teach students skills for developing portfolios and completing job applications.
    13. Coordinate with staff and the Pre-ETS counselor to implement the Pre-ETS program.
    14. Use community resources to enhance instructional programming.
    15. Provide effective guidance and technical assistance to staff.
    16. Secure in-school and community job placements for students ages 14-22.
    17. Building long-term relationships with employers and community organizations.
    18. Provide training and ongoing support to employers and job coaches.
    19. Support students in the completion of college and trade skill program applications.
    20. Coordinates transition activities for college readiness.
    21. Collaborate to develop a scope and sequence to ensure workplace readiness.
    22. Create and identify resources and training materials to use in professional learning and coaching sessions with staff.
    23. Provide students with the knowledge of public transportation options along with practice opportunities.
    24. Coordinating both on- and off-campus employment opportunities for current students.
    25. Coordinates transition activities for career and community readiness.
    26. Develop a scope and sequence to ensure students receive instruction in self-advocacy and self-determination.
    Physical Requirements: While performing the duties of this job, the employee is occasionally required to stand, sit, use hands to the finger(s), handle, or feel objects, tools or controls, use fingers to type and/or write; reach with hands and arms; talk or hear; taste or smell. Specific vision abilities required by the job include close, distance, and color vision, peripheral and depth perception and the ability to adjust focus. Must be able to lift, carry, push and pull up to 50 pounds. If lifting in excess of this weight is necessary, the assistance of a second person is required. The physical demands of this position will also include the ability of the individual to perform all of the essential functions and related responsibilities of the position with or without reasonable accommodations.

    Work Environment: This position normally performs the majority of work in a school building or work office environment.

    7. Perform other duties as may be assigned by the immediate supervisor(s).
