    Summit Healthcare Services is a holistic care management provider that brings together various disciplines to care for the aging population. Our Transitional Care Management evaluates individuals as a whole and supports aging in place through a combination of services, ultimately avoiding hospital readmissions and extended stays in skilled health facilities. Each community we partner with is assigned a dedicated team that establishes the Summit Center for Living Well, covering all six areas of care: Living Well, Private Care, Rehab Therapy, Home Health, Palliative, and Hospice Care.

    Role Description

    We're looking for a full-time Registered Nurse to visit patients in the East Valley for Summit Hospice. The Registered Nurse will be responsible for providing direct patient care, conducting assessments, developing care plans, administering medications, and coordinating with other healthcare professionals. This RN's caseload would be mainly in the Mesa/Gilbert/Chandler area, with the flexibility and autonomy to create one's own schedule and work how they prefer within the hours of 8-5p M-F.

    Qualifications

    • Current Registered Nurse (RN) licensure
    • Experience in providing direct patient care and conducting assessments
    • Knowledge of care planning and medication administration
    • Ability to coordinate with healthcare professionals and communicate effectively
    • Strong clinical skills and attention to detail
    • Ability to work independently and as part of a multidisciplinary team
    • Experience in geriatric care or related field is a plus
    • Bachelor's degree in Nursing or related field
