Home Health Clinician – Home Health
Schedule
Visits/hours self-scheduled based on HomeCare office needs. Every 3rd weekend required.
Requirements
- Minimum 2 years clinical experience
- Required: prior home health experience
- Valid driver's license and reliable vehicle for home visits
- Required certifications: BLS, state licensure (FL license or compact license required)
- OASIS training: preferred
- Familiarity with charting systems: BayCare Forms, BayCare Scheduling, Ace Timesheet; Cerner experience preferred
Roles & Responsibilities
- Conduct skilled nursing visits for patients in home settings, focusing on Cardiopulmonary, Neurology, Orthopedics, Oncology, and Geriatrics.
- Perform patient assessments and document findings accurately in compliance with BayCare policies.
- Develop and implement individualized care plans, including home exercise programs and patient education.
- Provide education and training to patients and families on managing health conditions and performing activities of daily living (ADLs).
- Monitor patient progress and adjust care plans as necessary to ensure optimal patient outcomes.
- Coordinate care with interdisciplinary teams and communicate effectively with healthcare providers.
Compliance / Notes
- COVID19 vaccine required; medical/religious exemptions accepted.
- Flu vaccine must be obtained or declination submitted; masks required in both cases while working.
- Compliance paperwork due two Fridays before start (10 days prior).
- Pass/fail EKG exam required (3 attempts allowed).
- Prestart modules required; recorded by facility.
