Social Service Aide
(Housing Navigator/Outreach worker)

Opening in San Bernardino and Rancho Cucamonga

Contract position

Pay range: $25.75-$27.51/hourly

Experience needed:

Must have experience in either Outreach work or Housing navigation

Associates degree in Social/Behavioral Science or Human Services

and:

One (1) year of experience providing healthcare, mental health, social or human services.

Experience in performing assessments, interviewing, referring clients to services and resources, crisis intervention

or:
Successful completion of a Community Health Workers/Promotes (CHW) program certificate

Job responsibilities include but are not limited to the following:

• Performing assessments
• Outreach to the public and/or landlords
• Conduct interviews
• referring clients to services
• Performing crisis interventions
• Create inventory of housing units
• Create budgeting
• Assist with applications for At Home Clients
• Provide landlord engagement
• Making calls and reaching out to various agencies, religious organizations, etc.

Must have reliable transportation, proof of Insurance and valid CDL
Pass background and drug test
