Compensation Range: $21/hr - $24/hr commensurate with experience.

Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. Overview:

Coordinate Intercambios Puerto Rico Clinic located in Fajardo (providing outpatient substance use disorder (SUD) treatment and COI as well as other health conditions.

Responsibilities:

  • Coordinate all program services to individuals who are experiencing substance use, homelessness, recovery and/or co-occurring mental health disorders.
  • Supervise personnel about task compliance, attendance, and performance.
  • Coordinates the monthly work plan.
  • Conduct weekly brief meetings to direct the work of the team.
  • Ensuring that evidence-based practices are properly addressed and documented.
  • Provide recommendations and follow-up if necessary.
  • Facilitate coordination and collaboration with other community-based organizations and partners.
  • Ensure program integration with other agency services.
  • Assist in the coordination of all data gathering, analysis, and program reporting.
  • Prepare Progress Reports.
  • Collect monthly reports from the team, monitor service delivery trends, identify areas for improvement and make a corrective action plan.
  • Oversee data entry (SUPRT, others) into the system and monitor project progress.
  • Responsible for directing the recruitment of new participants under the project and assigning them to the appropriate funding source.
  • Coordinate for the continuity of processes, licenses, and permits
  • Represent organization in different forums.
  • Support the work team in everything necessary for the continuity of services.
  • Support the evaluator or other contractors in the collection of information.
  • Report project progress or challenges to the project director

Requirements:

  • Experience with managing/administration of programs directed to people that have experienced homelessness, substance use, and/or co-occurring disorders
  • Experience with supervision of clinical and non-clinical staff.
  • Experience with Evidence Based Practices / Interventions
  • Knowledge about SUD, COI, HIV/AIDS, Hepatitis-C, and STDs in the Puerto Rican context
  • Highly motivated to work with people who experience homelessness, substance use, and/or co-occurring disorders in an outreach setting
  • Able to work closely with others in outreach team and office setting (team player)
  • Capable of executing a plan based on facts and scientific knowledge
  • Proficient and efficient with multiple computer software programs
  • Excellent written and verbal communications skills (in both Spanish and English a plus)
  • Meticulous at maintaining records and confidentiality protocols
  • Outreach experience a plus
  • Up to date driver's license
  • Master's Degree, preferably a LMSW, LCSW or LMHC (or related field). Live/work experience will be taken into account.
