- Support community-based COVID-19 response including outreach and education (e.g., flyering, tabling, mask distribution, community wellness activities).
- Build community trust through service-based work and relationship-building.
- Help people understand their health condition(s) and develop strategies to improve their health and well-being using Motivational Interviewing techniques.
- Deliver health information using culturally appropriate terms and concepts.
- Link people to health care and social service resources including health insurance, food, housing, quality care and health information.
- Provide informal counseling, support, and follow-up, and build individual and community's capacity to advocate for needs and services.
- Solicit and capture community feedback, particularly on community strengths and needs.
- Participate in group advocacy and community mobilization activities of the program and serve as a positive example and representative of the organization internally and externally.
- Document services and activities performed as required by agency and funder.
- Perform all functions in alignment with CHASI's Mission, Vision, and Core Values.

- High school diploma or GED required.
- Knowledge of NYC's resources including both governmental and non-governmental services.
- NYCHA residency required (due to funder requirements).
- Excellent problem-solving skills and crisis intervention techniques and motivational interviewing experience preferred.
- Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferred.
- Must have previous experience working with high-need priority populations.
- Ability to work 2 Saturdays per month required.
- Must be in good standing with NYC DOHMH and other city agencies.
- Strong attention to detail.
- A commitment to collective action and community organizing.
- Knowledge and familiarity with NYCHA systems is a plus.
- Knowledge of PHPC Salesforce system preferred.
- Bilingual (English/Chinese/Mandarin/Spanish/Haitian Creole) preferred.
- Ability to manage a caseload of up to 25 clients at a time.
- Experience using client relationship management (CRM) software is a plus.

Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.
POSITION SUMMARY:
The Vaccine Equity Partnership Engagement NYCHA COVID-19 Recovery Program (VEPE-NYCHA) will drive equitable pandemic recovery in public housing communities that have experienced longstanding health inequities stemming from historic and contemporary disinvestment and system racism. Community Ambassadors (CA) will engage with residents in identified NYCHA neighborhoods to increase vaccination and access to health and social resources, as well as increasing community capacity for collective action to improve health. CA's will be focused on increasing COVID-19 vaccination, addressing social and health factors that increase risk of severe COVID-19 illness, and leading community wellness programs. CA's will play a vital role in vaccination by educating communities with high levels of mistrust about the vaccine and facilitating access to the vaccine. The CA's will also support prevention, and management of chronic conditions, such as asthma, diabetes, hypertension, and obesity, that are known to make individuals and communities more susceptible to severe illness and death from COVID-19.
Note: This position includes working at least 2 Saturdays per month.
