Job DescriptionLicensed Clinical Social Worker needed for clinic in Durham, NC. Housing, travel, benefits offered to the qualified candidate. '..acute inpatient hospital located in Garner, NC. We serve adults and seniors struggling with mental health issues. We are at 56-beds capacity and growing to 72 beds. Our team provides structure therapeutic programming. We also ensure safe discharges through a coordinated effort with our community partners.
We are looking for clinicians who are motivated self-starters who want to be a part of a dynamic team serving some of the most vulnerable in our community.
Conducts a psychosocial assessment within 72 hrs. Develops and implements treatment plans for patient based on assessment and coordinates any additional services needed, revising, as necessary. Participates in multi-disciplinary treatment planning meetings. Presents appropriate patient information to the treatment team. Recommends effective treatment interventions. Maintains a caseload as assigned. Facilitates groups or program modules. Timely prepares progress notes and treatment updates in the medical record. Maintains all clinical documentation in accordance with regulatory and accrediting standards. May perform case management, discharge or after care planning as needed.
