We are interviewing for Advanced Practice Nurse Practitioners or Physician Assistants to join our rapidly growing House Calls / Mobile PCP practice. The current openings are in West Arlington/Fort Worth South of I30 area. Must live near the locations posted. Experience with Geriatric patient care in the home environment a plus. Only qualified candidates will be considered.

Nurse Practitioner Responsibilities:

• Performing physical exams and patient observations for Geriatric patients in their home, ILF, ALF, Memory Care, Group Homes and Senior Communities.
• Recording patients' medical histories and symptoms in EMR system.
• Creating patient care plans and contributing to existing ones.
• Ordering, administering, and analyzing diagnostic tests.
• Identifying the need for and ordering medical equipment.
• Diagnosing and treating acute and chronic health issues.
• Managing, ordering, assessing effectiveness of medications and other treatments.
• Detecting changes in patients' health and modifying treatment plans, as needed.
• Consulting with healthcare professionals and families.
• Training patients and their families to manage and prevent illness and injury.
• Quality Measures / MIPS data collection
