Job summary
Habilitation Coordinators assist individuals living in Nursing Facilities (NF) with intellectual and developmental disabilities in achieving their personal outcomes.The Habilitation Coordinator works with Nursing facility staff, legal guardians/actively involved family members and others to provide services and supports that will help the individual achieve a quality of life and community participation that is acceptable to them.
This position will also function as a back-up to the Enhanced Community Coordination/Diversion Coordination (ECC) and Intellectual and Development/Mental Illness (IDD/MI) PASRR Evaluators.
