- Reviews new Spine Center referrals with clinical teams to determine the appropriate avenue for care. Ensures New Spine Center patients are scheduled appropriately and achieves New patient access goals.
- Communicates effectively with patients, Spine Center clinicians, and staff. Works effectively across various teams.
- Establishes a caring therapeutic relationship with patients and families utilizing effective communication while managing a defined patient caseload.
- Assesses patients for social determinants of health, utilizing findings to develop a plan of care and mutual goal setting.
- Serves as liaison for the patient, becoming an expert on the patient and their needs throughout the trajectory of their care.
- Advocates for patient and their families to ensure timely access to treatment and equal access to healthcare.
- Collaborates with all members of the healthcare team in a positive professional manner to ensure coordination of patient care.
- Provides psycho-social support and facilitates referrals and transitions of care ensuring effective utilization of internal and community resources.
- Provides symptom managements utilizing guidelines/evidence based practice from resources in collaboration with providers.
- Coordinates/schedules admissions, surgeries, imaging, laboratory testing, etc.
- Assesses patient and family learning style to provide ongoing education regarding diagnosis, staging, treatment, side effects, and self-care.
- Prioritizes complex patient care based upon knowledge and clinical experience.
- Participates in quality metric initiatives supporting optimal outcomes and value-based metrics.
- Assists in patient identification for tumor board, coordinates tumor board, and participates in patient presentation and care plan development.
- Serves as key member of multi-disciplinary team supporting patient cohesiveness, efficiency, and patient focused care.
- Other duties as assigned.
- Bachelor's degree in Nursing or Master's degree in Nursing required.
- Current State of Illinois Registered Nurse licensure required.
- At least three years of nursing experience required.
- Strong verbal and written communication skills.
- Ability to work on a team.
- Ability to use a keyboard to enter and retrieve data.
- Ability to travel throughout the Medical Center and some roles may require travel to other sites of care.

Summary:
The Spine Center Nurse Navigator is a registered nurse who works with physician practices and multidisciplinary teams to promote the effective utilization of services and coordination of care for patients within the Spine Center, which consists of Neurosurgery and Physiatry providers.
The Nurse Navigator contributes to the assessment of patients' psychosocial, financial, and clinical needs throughout the continuum of care ensuring high quality care while supporting quality metrics.
The Nurse Navigator functions as a liaison between patients, physician practices, the hospital, and the community performing functions that meet the core competencies navigation.
