- Implements client individual action plan, objectives and methods consistent with assessment of problem
- Coordinates services with other staff and other agencies
- Contacts collaterals and family members to encourage their participation in client's treatment as well as their own treatment, if needed
- Serves as resource person/consultant to other service providers in identifying clients for referral and works cooperatively with other providers to assure linkage of clients with appropriate services
- Prepares reports, maintains clinical files, participates in training and provides statistical materials
- Assures that advocacy, linking, expediting and related activities are carried out as needed
- May provide site based or community based services
- Performs other duties as assigned
- Must be trained and demonstrate a basic knowledge and understanding of working with populations, individuals diagnosed with severe mental illness or substance use disorders
- Must be proficient in the use of computers and computer software. Familiar with all Microsoft products
- Training in Motivational Interviewing and possess an understanding of domestic violence
- High School Diploma and one to three (1-3) years of experience in a Mental Health or Human Services setting
- Preferred Associate's or Bachelor's Degree and one (1) year experience
- One to three years in Mental Health or Human Services setting dependent on certification/education

SUMMARY OF POSITION FUNCTION:
Provides Psychiatric Rehabilitation Services to individuals with substance use disorders, mental health disorders and/or co occurring disorders. Provides Individual and Group Psychiatric Rehabilitation services. Also is responsible for referral to other appropriate services and coordination with other service providers. Performs related tasks as required.
