We are now seeking a Nurse Practitioner to join our Healthcare Group in the Primary Care division. The candidate will care/treat patients in a Long-Term Care facility in Glendale, WI. Our clinicians are filled with high energy and excitement. We look forward to introducing you to the team

Responsibilities:

• Examine patients and their medical records
• Prescribe medications
• Maintain accurate records and schedules
• Inform patients about their health conditions and prescribed medications
• Educate patients and their family members on diseases, epidemics and prevention
• Oversee patients' overall health
• Promote good health practices
• Advance professional knowledge and conduct research
• Propose treatment plan for Annual Wellness visits

Requirements:

• Candidate should possess a valid NP license in the state of Wisconsin
• Candidate should be OK with nursing home setting

Schedule:

• 3 days per week
• 8 hour shifts
