    TITLE: ACCS Lead Clinician

    PROGRAM: Adult Community Clinical Services (ACCS)

    SUPERVISOR: ACCS Team Director

    Company Overview: From humble beginnings with a community-based Board of Directors and a budget of less than five hundred thousand dollars, the center has expanded its services to support individuals with mental health conditions, intellectual and developmental disabilities, co-occurring disorders, autism, and brain injuries.

    The Center's clientele spans a wide age range, from three to eighty-seven years old. While historically serving both adults and children, there's a noticeable increase in Transition Age Youth referrals, prompting a responsive approach to their specific needs. With a dedicated team of approximately 400 full and part-time staff members, the we serves over 3,000 individuals annually.

    MINIMUM QUALIFICATIONS:

    Master's degree in relevant clinical field. Strong preference for a Licensed Practitioner of the Healing Arts (LICSW, LCSW, LMHC) and at least 2 years of experience working with adults with mental health concerns. If not currently a LPHA, then will become LPHA within 2 years, but must have at least five years of experience working with adults with major mental illness and co-occurring substance use disorders. Experience working as part of a multidisciplinary team in the provision of clinical, rehabilitative, and recovery oriented services to Persons served with major mental illness preferred. Training and experience in Dialectical Behavior Therapy and/or Cognitive Behavioral Therapy, Illness Management and Recovery, Stages of Change, Screening, Brief Intervention, Referral, and Treatment (SBIRT), Substance Use Treatment, Housing First, Harm Reduction, and Motivational Interviewing strongly preferred. Valid driver's license and reliable vehicle required. Stages of Change, Screening, Brief Intervention, Referral, and Treatment (SBIRT), Substance Use Treatment, Housing First, Harm Reduction, and Motivational Interviewing, First Aid, CPR, and MAP certification within three months of hire required.

    OVERVIEW: Supervise designated ACCS clinicians and interns. Responsible for providing clinical services to Persons served. Maintains a roster of Persons served receiving integrated team/ GLE services. Oversees the primary teams and develops assessment, treatment plans, and reviews. Delivers interventions identified on the Treatment Plan and assessment. Provides direct intervention with Persons to conduct comprehensive assessments, Treatment Plans. Provides modeling and supervision for direct care staff. Provides trainings and leads discussions for other ACCS staff.

    RESPONSIBILITIES:

    A. Treatment Planning and Provision

    1. Provides planning, coordinating and documenting screenings with Persons served.

    2. Establishes and maintains therapeutic, supportive relationships with all Persons served as the primary clinician for persons living in the community and in GLE's.

    3. Acts as primary clinician for persons in the integrated team/ GLE's.

    4. Responsible for identifying engagement strategies, considering the Person's situation and preferences, which are utilized by the clinician and the full team.

    5. Completes the Persons' assessments and Treatment Plans, including face-to-face meetings with the Person with the assistance of the other integrated team/GLE staff members.

    6. Responsible for ensuring that interventions are informed by evidence-based strategies and Integrated Team/ GLE staff members delivering these interventions are adequately trained and supervised. 7. Participates in transition planning meetings as organized by DMH or other youth service agencies. 8. Conducts the assessment and Treatment Plan utilizing a developmental approach and selects interventions appropriate to the Person's age, interest and abilities.

    9. Develops and implements the planning, coordinating, and documenting the treatment planning process with Persons served as assigned, using recovery-oriented, person centered, and evidence-based practices. 10. Delivers treatment interventions, including strategies related to Motivational Interviewing, SBIRT, Harm Reduction, Stages of Change and Housing First, CBT, DBT, and other evidence-based practices.

    11. Implements principles of trauma-informed care, person-centered approaches, and recovery-oriented services and supports.

    12. Provides persons support and treatment related to health and wellness, crisis prevention and intervention, culturally, linguistically and age appropriate policies and practices, substance use/addiction (signs, techniques to support recovery, resources), and housing related needs.

    13. Incorporates a Person- and, when relevant and requested, a Family- Centered Approach into all assessments, treatment planning, interventions and interactions with Persons served.

    14. Develops Crisis Prevention Plans, Safety Plans, or Wellness Plans with Persons served, in conjunction with the integrated team/ GLE, other providers and natural supports as desired by Person served.. 15. Provides support and treatment through care transitions and other life transitions.

    16. Promotes Overall Health and Well-being by addressing the Eight Dimensions of Wellness, identified by SAMHSA: Emotional, Environmental, Financial, Intellectual, Occupational, Physical, Social and Spiritual. 17. Involves Person's Legally Authorized Representatives in planning.

    18. Involves Person's family and significant others in planning where desired by Person served. 19. Teaches and demonstrates good interpersonal skills, maximizing community resources, and advocating for a person's fiscal entitlements.

    20. Teaches and assists with activities of daily living, i.e. hygiene, housekeeping, shopping, cooking, budgeting, laundry, etc.

    21. Appropriately and effectively role models for Persons served through actions and appearance. 22. Assists Persons served in gaining social competencies in the community that will enhance job acquisition and tenure.

    23. Administers or monitors self-administration of medication, while teaching maximum independence, as assigned.

    24. Provides education, support, and assistance to Persons served in the areas of peer-to-peer support services, WRAP, and other self-help recovery strategies.

    25. Utilizes and teaches problem-solving techniques.

    26. Assists in identifying and facilitates the use of appropriate self-help resources.

    27. Conducts housing searches for Persons served, as assigned.

    28. Assists Persons served in attending activities and appointments, providing transportation when appropriate. Teaches and assists Persons served with the maximum use of public transportation, as needed. 29. Provides crisis intervention as needed, consistent with Crisis Prevention/Intervention, Safety Plan and/or Wellness Plans and Center policies and communicate with Care Coordination Entities, local Emergency Services Providers (ESPs) and other providers.

    30. Responds to emergencies appropriately.

    31. Administers first aid or other emergency assistance as necessary

    32. Provides back up for other ACCS staff as necessary.

    33. Provides accurate observations/assessments of Person served behavior.

    34. Observes and documents Person served progress toward meeting objectives.

    35. Monitors and records Person served progress in all service areas.

    36. Respects Person's rights, preferences and choices in all aspects of service delivery.

    37. Ensures all clinical records are completed according to established guidelines and timelines.

    38. Ensures communication with all relevant parties including family members, significant others, care coordination entities, and other providers.

    B. Program Operations and Planning

    1. Provides Directorship in ongoing program Quality Improvement system.

    2. Participates in and ensures compliance with the DMH Contract Monitoring system.

    3. Ensures adherence to Center/ACCS Services policies and procedures.

    4. Ensures adherence to the program's recovery philosophy.

    5. Ensures compliance with all state and local licensing/certification requirements and regulations. 6. Ensures compliance with DMH Rehabilitation Option standards.

    7. Coordinates planning efforts with agency managers, other ACCS managers, administrative staff, outside agencies and DMH personnel.

    8. Remains awake and alert at all times during all shifts.

    C. Staff Management

    1. Provide direct supervision, side-by-side supervision, mentoring, support, training, and performance evaluations to assigned ACCS clinicians and interns.

    2. Provides mentoring, support, and training to all program staff.

    3. Participates in development of staff in-service training and development.

    4. Participates in the interviewing and hiring process of personnel.

    5. Take disciplinary action as necessary according to established policies and procedures. 6. Provides on-call coverage as assigned.

    D. Team Participation

    1. In coordination with the Team Director and Assistant Team Director, takes a lead role in weekly staff meetings.

    2. Participates in monthly ACCS Administrative meeting.

    3. Participates in ACCS leadership meeting as needed.

    4. Participates in monthly All Management meeting.

    5. Leads treatment meetings.

    6. Attends and participates in all other meetings as scheduled.

    7. Meets weekly with the Team Director for supervision. Seeks and uses additional supervision appropriately.

    8. Attends and participates in Center/DMH/other agency taskforces/committees as assigned. 9. Communicates ideas effectively to other team members.

    10. Demonstrates a positive attitude, open-mindedness and flexibility to incorporate new information.

    E. Administrative

    1. Participates in proposal writing and presentation as assigned.

    2. Participates in PR development and presentation.

    F. Professional Development

    1. Attends relevant seminars, workshops, conferences and internal trainings.

    2. Consults with other professionals, as appropriate.

    3. Keeps abreast of and shares relevant current literature.

    G. Other Job Responsibilities

    1. Performs other job duties required and consistent with the nature of the job.

    Job Type: Full-time

    Salary: $68, $75,000.00 per year
