    About This Role

    As a Registered Nurse (RN) in our Hospice program, you will provide compassionate and holistic care to patients and their families. Your expertise and dedication will make a meaningful difference in the lives of those we serve.

    Key Responsibilities:

    • Deliver nursing care to assigned patients using the nursing process and adhering to standard nursing practice and established policies and procedures.
    • Coordinate patient care, supervising LPNs/LVNs and Home Health Aides as needed.
    • Collaborate with other disciplines to provide optimal patient care and services.
    • Develop and revise individualized Plans of Care in partnership with patients and caregivers.

    Clinical Experience and Qualifications:

    • Graduate from an accredited school of nursing.
    • IL Licensed Registered Nurse required.
    • Accreditation Board for Specialty Nursing Certification in Hospice and Palliative Care preferred.
    • Valid IL driver's license and a clean driving record.

    Clinical Advancement Opportunities:

    • Clinical Nurse I: 0-9 credits on the Clinical Ladder Behavior Grid.
    • Clinical Nurse II: 10-19 credits on the Clinical Ladder Behavior Grid.
    • Clinical Nurse III: 20-29 credits on the Clinical Ladder Behavior Grid.
