    Description

    Clinical Care Specialist

    As a skilled Clinical Care Specialist, you will be the cornerstone of patient care at our esteemed healthcare institution. Your dedication to delivering exceptional service and compassion will have a profound impact on the lives of our patients, their loved ones, and the community we serve.

    • Key Responsibilities:

      • Patient assessment and care planning utilizing evidence-based practices and interdisciplinary collaboration.

      • Development and implementation of individualized treatment plans tailored to each patient's unique needs and goals.

      • Effective communication and documentation of patient progress, collaborating with healthcare teams to ensure seamless care transitions.

      • Compliance with organizational policies, procedures, and protocols governing safety, medication administration, and documentation.

    • Required Qualifications:

      • Diploma or Associate's Degree in Nursing.

      • Registered Nurse license in New York State.

      • Basic Life Support (BLS) certification.

    Educational Requirements:

    • AS: Nursing (Required)

    Licenses/Certifications:

    • BLS - Basic Life Support - American Heart Association (AHA)

    • RN - Registered Nurse - New York State Education Department (NYSED)

    Physical Requirements:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
