    Responsibilities

    • Average of 15-18 patients per day; no nights or weekends
    • Shared call rotation among providers on weekends
    • Work collaboratively to improve community health
    • Evaluate, interpret, and coordinate information for patient care decisions
    • Patient-focused, friendly, and team-oriented
    • Excellent communication, analytical, and problem-solving skills

    Qualifications

    • Nurse Practitioner or Physician Assistant
    • Board Certified
    • 3-4 years of experience preferred
    • Hold an unrestricted medical license in Illinois
    • Hold current state and federal authority for prescribing medications
    • Eligible to work in the United States
