    Summary:
    The Prescription Assistance Program Associate works collaboratively with the health care team in determining if patients would qualify for assistance with their medications. The Associate receives referrals, assists the patient in filling out applications for medication assistance and follows the application to ensure pharmaceutical assistance programs are processing the application. The Associate communicates with the patient regarding the approval of assistance and also offers alternative resources for those who do not receive approval.

    Essential Job Functions:
    • Completes documentation/paperwork for patients regarding pharmaceutical assistance programs.
    • Maintains computer program related to Prescription Assistance Program.
    • Coordinates the medication renewals and ensures follow-up is completed between the physician/provider and the patient.
    • Ensures screening is completed regarding referrals to Social Work.
    • Performs other duties as assigned or needed to meet the needs of the department/organization.
