- 13 to 26 weeks
- Conducting intake assessments with patients and families.
- Ensuring that families feel sufficiently equipped and supported.
- Developing individualized treatment plans that account for physical, financial, and other constraints.
- Adjusting treatment protocols, as needed.
- Suggesting and coordinating appropriate follow-up services to promote wellness.
- Manage a caseload and complete face to face visits on members periodically.
- Meeting with members of healthcare teams to discuss patients' progress, and to deliberate on appropriate care.
- 2+ years of pediatric clinical nursing experience - Required.
- Active RN License of OH- required.
- Must live in OH – Northeast region.
- Bachelor's degree in nursing- required.
- Bilingual in Spanish and English - preferred.
- At least basic Microsoft office experience
- 8:00 AM - 5:00 PM EST
- Monday - Friday
- 5x8
- 8 hour shift
- Monday to Friday
- Must live in OH – Northeast region
