    JOB SUMMARY

    The Clinician – (ASW, AMFT, APCC) Mobile Outreach ensures services are effective and timely. performs a range of psychotherapeutic and rehabilitative interventions including psychosocial assessment, individual and group counseling, crisis intervention and limited case management duties as needed. In addition, the Clinician serves as the Care Coordinator for clients ensuring that they receive the full spectrum of services in a seamless manner and that they receive the optimal level of care needed to maximize functioning and minimize the need for higher levels of care.

    • Works closely with Psychiatrist to monitor the psychiatric well-being of clients.
    • Provides field-based services in partnership with the Peer Support Specialist – Mobile Outreach.
    • Practices flexibility by meeting client needs quickly and creatively.
    • Works closely with community providers to meet the needs of the homeless
    • Utilizes creative means and evidence-based practices to engage clients who are high risk, disengaged, and unconnected.
    • Serves as Care Coordinator for a caseload of clients as assigned by Program Manager.
    • Provides a variety of services including group and individual counseling, rehabilitation/skills building, relapse prevention,

    and crisis intervention.

    • Performs psychosocial assessments. Practices the principles of Psychosocial Rehabilitation and Recovery.
    • Demonstrates and supports a strong working knowledge of the CCISC model of dual recovery principles and practices,

    applying an integrated treatment approach with PSR principles.

    • Interacts therapeutically with clients and exhibits boundaries consistent with professional standards.
    • Works with volunteers and interns in an informative and collegial capacity.
    • Completes all charting and documentation as contractually required and in timely manner as dictated by program

    requirements. Assists in promoting program services to community when appropriate.

    • Participates in the Injury and Illness Prevention Program and follows guidelines that promote workplace safety.
    • Provides input towards policy and procedure design, revision, and deletion.
    • Ensures facility and program upkeep and safety by maintaining neat and clean work areas, by securing work area at close

    of business day

    • Comply with all MHS and program rules, policies and procedures including HIPAA standards.

    QUALIFICATIONS

    • Bilingual in one of the threshold languages preferred.
    • Master's Degree in Social Work, Psychology, Counseling, or related field.
    • BBS licensed in the state of California as a LCSW, LMFT or LPCC required
    • Two years of experience working with mentally ill populations.
    • One year of experience working with acute psychiatric and dually diagnosed individuals is preferred.
    • Knowledge of Best practices in psychiatric, substance abuse, psychosocial rehabilitation, and recovery, and the CCISC

    model of integrated services for co-occurring disorders.

    • Knowledge of Medical model including the DSM, psychosocial rehabilitation and CCISC model.
    • Extensive knowledge of individual, group, family, and cognitive therapy techniques.
    • Knowledge of Cultural and socio-economic factors that inform service provision to persons with psychiatric or dual

    diagnosis disorders.

    • Knowledge of Current Diagnostic and Statistical Manual of the American Psychiatric Association.
    • Knowledge of Symptoms and treatment of people suffering from co-occurring mental illness and substance abuse

    disorders.

    • Knowledge of Behavioral management techniques, crisis intervention skills; assessment, diagnosis, and treatment

    techniques. Knowledge of Laws and ethics guiding the profession.

    • Proficient user of Microsoft Office Suite and EHR software.
    • Ability to problem solve and react appropriately to emergency situations.
    • Ability to work independently and effectively collaborate with a team.
