    Job Summary:
    We are seeking compassionate and experienced Registered Nurses (RNs) to join our Home Care team. As an RN in Home Care, you will have the opportunity to provide skilled nursing care to patients in the comfort of their own homes. The ideal candidate will have strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality care to patients of all ages across the lifespan.

    Responsibilities:

    • Conduct comprehensive assessments of patients' health status, including physical, psychological, social, and environmental factors, to develop individualized care plans.
    • Provide skilled nursing care, including wound care, medication management, IV therapy, and patient education, in accordance with physician orders and established protocols.
    • Coordinate with interdisciplinary team members, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and coordinated care for patients.
    • Monitor patients' condition and response to treatment, adjusting care plans as needed to promote optimal outcomes and patient comfort.
    • Educate patients and families about their health conditions, treatment options, and self-care techniques, empowering them to participate in their own care and make informed decisions.
    • Document all patient assessments, interventions, and outcomes accurately and in compliance with regulatory requirements, using electronic medical records systems.
    • Communicate effectively with patients, families, and healthcare team members to ensure continuity of care and promote patient safety and satisfaction.
    • Adhere to infection control protocols, safety guidelines, and organizational policies to maintain a safe and healthy environment for patients and staff.

    Qualifications:

    • Valid Registered Nurse (RN) license in the state of practice.
    • Minimum of two years of clinical nursing experience, with experience in home health, community health, or hospice nursing preferred.
    • Strong assessment, communication, and interpersonal skills.
    • Ability to work independently and collaboratively in a dynamic and changing environment.
    • Commitment to providing compassionate and patient-centered care to patients in their homes.
