    Description

    Adoration Home Health and Hospice - LPN Job Opening in Chattanooga Branch

    • Supporting Home Health patients under RN supervision
    • Providing routine care and monitoring client's health status
    • Assisting in treatments and communicating with clients and families

    Responsibilities:

    • Providing 1:1 care following client's plan of care, maintaining dignity and independence
    • Monitoring vital statistics and reporting findings to Case Manager and Physician
    • Administering medications, feedings, oxygen, ostomy care as needed
    • Completing routines established by care professionals
    • Ensuring infection control policies, reporting possible diseases, maintaining procedures
    • Documenting nursing actions, progress, outcomes, and medical condition changes
    • Coordinating supplies, medications, and ensuring patient needs are met
    • Monitoring incidents, completing reports, and examining individuals
    • Maintaining medical equipment
    • Other duties as assigned

    Qualifications:

    • LPN/LPT/LVN Degree with current license in state of practice
    • Valid driver's license, acceptable driving record and proof of car insurance
    • CPR professional certification
    • Experience in administering medications
    • 1+ years of nursing experience preferred
    • Home health care experience preferred
    • Able to lift and carry items weighing up to 20 pounds
    • Able to bend, stoop, push, pull, sit, and walk for periods of time
    • Must communicate verbally and in writing
