    Job Title: Full-Time Home Health Registered Nurse

    Responsibilities

    The RN in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

    • Coordinates Total Plan of Care: Collaborates with other healthcare professionals involved in care to achieve continuity of patient care
    • Makes Initial & Comprehensive Nursing Evaluations: Determines primary focus of care and develops plan of care within state-specific guidelines
    • Ensures Patient Eligibility: Verifies patients meet home health eligibility and medical necessity guidelines defined by payer source

    Education and Experience

    • Current RN Licensure in PA
    • Current CPR Certification
    • Valid Driver's License, Vehicle Insurance, and Access to Dependable Vehicle or Public Transportation
