No need to submit another application if you've already applied to this position Applicants will be reviewed weekly starting 1/29/24.We're seeking a compassionate, holistically-inclined behavioral health and substance use disorder professional to join our team as the LGBTQIA2S+ Clinical Services Specialist
    • Care Coordinator.
    The Care Coordinator uses a fidelity team-planning process to meet the needs of our community's most vulnerable individuals impacted with complex behavioral health needs including co-occurring Substance Use Disorder (SUD) and Mental Health (MH) Disorders as well as multi-system involvement and are oppressed and marginalized.

    In this role, you will provide client centered, trauma informed clinical assessments, analysis, treatment services, specific engagement, care coordination, and case management to eligible LGBTQIA2S+ individuals with complex behavioral health needs.

    In addition, it provides advanced human services clinical assessment, analysis and/or treatment services involving complex psycho-social factors, and coordination of specific case plans to meet medical and psycho-social needs of economically, physically, developmentally, mentally or socially disadvantaged clients and their families.


    Knowledge, Skills and Abilities:

    This position requires extensive knowledge of and experience working with the LGBTQIA2S+ community, as well as broad knowledge of culturally matching community resources.

    This role provides outreach, engagement, and care coordination to help LGBTQIA2S+ youth and families overcome barriers to health care.

    The role also requires an in-depth understanding of how to support the needs of LGBTQIA2S+ people with a multitude of intersecting identities (ex: BIPOC, immigrant/refugee, differently-abled, etc).

    This is demonstrated by knowledge of cultural norms within the LGBTQIA2S+ community, as well as having a close affiliation with and lived experience in the LGBTQIA2S+ community.


    Teleworking:


    At this time, this position is designated to "routine telework," meaning you will be working remotely (from home and community) most of the time; a hybrid schedule also is available.

    As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

    As the LGBTQIA2S+ Clinical Services Specialist
    • Care Coordinator you will:Assesses, develops and integrates a care plan for complex and difficult cases with other members of the Care Coordination Services team, members and their support systems, other community based partners and clinical care providers.
    Provides community based resources and services to LGBTQIA2+ adults.

    Provides outreach, engagement, supportive services as well as access to services for participants through intensive care coordination and case management activities.

    Determines need for appropriate assessment tools for comprehensive neuropsychological, psychological, psychiatric, psycho-diagnostic, or clinical case consultations with other qualified professionals.

    Builds and engages working relationships with other members of the care team, including clients and their support systems and other external partners.

    Assists members in gaining access to a variety of social services programs as needed, including SUD treatment, mental health treatment, housing, peer support, medical services, etcProvides consultation to community members who contact the Care Coordination Services Team seeking assistance for friends or family.

    Maintains timely and accurate notes on members served within the assigned Electronic Health Record system to track care, service plans per member, next steps or missing resources needed in care plan, etc.

    Completes notes as needed for billing and grant funding for any services provided and coordination of care between and with member organizations.

    Enters documentation into tracking sheet and/or electronic medical record to record outcomes or other data per team work flows.

    Enters, controls, maintains and reports out as appropriate investigations conducted, clients with urgent or specialized needs that may not fit into standard practice or system of care, coordinates findings with team members as part of investigation team or as internal resource, potential violations of policies or processes from internal and external partners, and works with program leadership on management of urgent issues for reporting requirements.

    Required Minimum Qualifications/ Transferable Skills*:

    Education/Certification:

    Masters Degree; must meet the standards of Qualified Mental Health Professional - (QMHP-C) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website.

    Current QMHP-C and/or licensed Board of Oregon (LPC, LCSW, PHD,etc) (Required)

    Graduate degree in psychology orGraduate degree in social work or counseling orGraduate degree in a behavioral science field, ANDExperience:
    Two (2) years of professional work experience as a counselor, Mental Health treatment professional, social worker, or therapist working with individuals suffering from substance use disorder (SUD)

    Preferred Qualifications/Transferable Skills:
    Addiction Counselor Certification
    • I (CADC I), Board of Oregon
    • Certified Alcohol and Drug CounselorOther: Valid Driver's LicenseThis position requires a Background Investigation, which may include being fingerprinted.
