    The Children's Home of Reading Youth & Family Services, is seeking a Full-Time, First Shift,Residential Case Manager to provide quality therapeutic case management services for adolescents and their families as part of a residential / day treatment team. The primary office hours are typically from 9AM- 5PM with one evening required weekly and occasional weekends. This is not a remote position.

    Primary responsibilities: Act as a liaison and maintain communication between The Children's Home of Reading Youth and Family Services, the primary caregivers, and referral/other agencies. Represent the agency at court hearings and interagency meetings. Provide life skills education, coordinate care services, and assist with discharge planning for residents.

    Education and Experience:
    • Must be at least 21 years of age.
    • Bachelor's Degree in social work, psychology or other related field REQUIRED
    • One to three years of experience working in a human services field (CASSP system-related setting preferred).
    • Experience with adolescents and mental health preferred
    Additional Requirements: The candidate must pass a Criminal Record History Clearance, FBI and Child Abuse History Clearance, as well as possess a valid PA Driver's License. The candidate must pass a Physical & Drug Screening & possess the ability to attend and successfully complete all sponsored trainings.
