Roles & ResponsibilitiesEvaluate patients and create individualized, goal-driven treatment plans
Deliver skilled physical therapy interventions to promote safe mobility
Document clinical progress clearly and timely in the medical record
Collaborate with nurses, OT, SLP and case managers to optimize patient outcomes
Educate patients and families on home programs and safe discharge plans
Contribute to a positive, team-focused culture that prioritizes patient-centered care
Qualifications
Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — DPT/ PT credential indicated in the job skills
New grads welcome — Years of experience: any, including New Grad
Comfort working in a Skilled Nursing Facility (SNF) practice setting
Strong communication skills and ability to work collaboratively with interdisciplinary teams
Current CPR/BLS recommended (confirm facility requirement upon interview)
Education
DPT or PT degree (Physical Therapy). Job indicates PT / DPT credentials.
Knowledge & Skills
Skilled in patient assessment, mobility training, and therapeutic exercise
Knowledge of documentation best practices and care plan development
Able to perform safe transfers and fall-risk mitigation strategies
Good clinical reasoning, time management, and patient education skills
Comfortable working with geriatric and post-acute populations
