Description
Liberty Cares With Compassion At Liberty Home Care, we understand that recovering at home after an illness, trauma, or surgery can significantly enhance patient outcomes. Our healthcare professionals are committed to providing patients with the opportunity to recover with independence.
We are currently looking for an experienced:
HOME CARE CLINICAL MANAGER - RN Full Time
Position Overview:
- Coordinate and manage all patient services
- Document patient care and staff performance effectively
- Supervise assigned Care staff
- Support the Administrator as needed
- Deliver direct patient care including case management, Oasis care planning, staff assignments, and optimal utilization of existing resources
Qualifications:
- Active RN license in the state of branch location and completion of nursing education from an accredited school
- Five (5) years of diverse clinical nursing experience with at least three (3) years in home health (preferred)
- Thorough understanding of homecare regulations
- Proficiency in organizing work independently, providing effective case management, delivering cost-efficient home healthcare services, and maintaining accurate documentation
- Strong critical thinking and communication skills in both oral and written English
- Proficiency in computer usage with knowledge of various applications (e.g., Word, Excel, Windows)
- Minimum of twelve hours of continuing education annually
- Leadership background is a plus
- CPR certification required
