Assesses client needs for social services, provides referrals, and facilitates client access to care by determining eligibility and facilitating enrollment in appropriate health insurance programs, including MDPH Medical Case Management, HIV Drug Assistance Program, and MassHealth. Coordinates service delivery, and advocates for community-based social services for TB, HIV, and harm reduction.
Major Responsibilities:
• Working as an integrated member of the harm reduction and HIV primary medical care team, meets with clients, facilitates access to and coordinates delivery of social services, including financial assistance, health insurance, medication, reimbursement, transportation, and housing.
• Provides information and referral to specialized intervention programs, including substance abuse detoxification and rehabilitation. Advocates and works to resolve problems of client access to these services. Follows up with clients to evaluate social service referrals, working with medical and mental health providers as well as community service agencies to develop alternate plans when indicated.
• Functions as a Case Manager/Liaison among service providers, both within the institution and the community. Maintains ongoing communication/collaboration with harm reduction and HIV Clinical Center team members, acting as a resource and referral source.
• In coordination with the outpatient clinic scheduler and providers, identifies clients who are uninsured or need medication coverage. Meets with and counsels' clients to determine their eligibility for MassHealth, obtaining data regarding their medical and social history, income and family status and insurance status. Enrolls clients who are ineligible for MassHealth insurance or who have gaps in coverage by utilizing the HDAP program.
• Makes appointments to renew client enrollment in this program annually. Completes forms as required by the MDPH and maintains program enrollment data, both in written and electronic form.
• Educates patients and families about harm reduction, HIV, and HCV, medications, positive prevention techniques and appropriate medical follow-up. Monitors adherence to treatment plan. Develops referral mechanisms.
• Attends UMMHC HIV and harm reduction team meetings to improve program. Receives training at least annually to stay abreast of changes in the HIV, HCV, STI, SUD and harm reduction fields and in resources as they affect the social service needs of infected patients.
• May be responsible for gathering and reporting consumer (patient) input and suggestions. Coordinates the Annual Patient Satisfaction survey, tabulates, analyze the results, and reports back to the committee.
• May be responsible for Quality Management of HDAP applications. Assists in getting the HDAP application approved in a timely manner.
• May assist with data entry and review, and other administrative tasks as necessary.

• Bachelor's Degree in Social Work, Human Services, Counseling, or a related field.
Experience/Skill:
Required:
• Two years of social work, human service, or related professional experience, including demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, and ability to work in a multi-disciplinary team.
• Ability to communicate clearly and concisely in both oral and written form.
• Must possess problem solving skills.
• Ability to work in multi-cultural environment.
Preferred:
• Experience with principles of health equity.
