The Care Transition Coordinator is responsible for the identification and assessment of home care needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.
The Care Transition Coordinato r will be based out of Sentara Albemarle Medical Center in Elizabeth City, NC.
Shift: Monday - Friday (8am-5pm)
