    Clinical Pharmacist

    Position Description:
    Complies to the standards of pharmacy operation Assumes responsibilities for own professional education and assists in providing education to patients and other health care professionals Utilizes pharmacy operation standards as outlined in policies and procedures pursuant to patient specific services that affect patient outcomes, optimize drug therapy and minimize costs Utilizes effective communication and positive interpersonal skills Demonstrates competence on a consistent basis in regards to medication dispensing Performs duties in an independent manner and demonstrates desire to learn new tasks with little or no supervision Critically evaluate the patient#s drug regimen in context of the patient#s medical conditions, available laboratory data and other pertinent patient parameters Accurately detects, reports and monitors potential drug related problems Provides rational drug therapy recommendations based on information from the medical record, laboratory data, patient interview and other pertinent patient parameters Is able to formulate an optimal pharmacotherapeutic plan which organizes and prioritizes interventions listing options and providing follow-up recommendations #

    Experience/Qualifications:

    Clinical Pharmacist


    Position Description:
    Complies to the standards of pharmacy operation

    Assumes responsibilities for own professional education and assists in providing education to patients and other health care professionals

    Utilizes pharmacy operation standards as outlined in policies and procedures pursuant to patient specific services that affect patient outcomes, optimize drug therapy and minimize costs
