The Discharge Planner develops coordinates and implements an appropriate discharge plan for a specific caseload of patients collaborates with the patient family and members of the multidisciplinary healthcare team to ensure safe effective continuum of care beyond patient's hospital visit promotes safe timely discharges with focus on patient satisfaction financial efficiency assists patients families by thoroughly answering all questions regarding patient's visit as well as those involving future healthcare services.Responsibilities
