    Join Our Team as an MDS Coordinator

    We are seeking a dedicated
    MDS Coordinator to oversee the Minimum Data Set (MDS) process for our Senior Living Facility residents.

    As an integral member of our team, you will collaborate with healthcare professionals to ensure accurate and timely completion of assessments and documentation, while ensuring compliance with CMS regulations.

    MDS Coordinator Essential Duties and Responsibilities:

    • Coordinate and manage the MDS process for all SNF residents
    • Collaborate with healthcare team to complete assessments and care plans accurately and on time
    • Analyze MDS data to identify trends and implement quality improvement initiatives
    • Ensure compliance with CMS MDS regulations
    • Provide staff training on MDS procedures and requirements

    MDS Coordinator Qualifications:

    • Registered Nurse (RN) with 2+ years of experience in a skilled nursing facility
    • Minimum 2 years of experience with the MDS process
    • Strong knowledge of CMS MDS regulations
    • Excellent organizational and communication skills
    • Ability to work independently and collaboratively

    MDS Coordinator Preferred Qualifications:

    • Bachelor's degree in nursing or related field
    • MDS certification
    • Experience with MDS software programs
    • Knowledge of quality improvement initiatives
