    Job Summary:

    The RN Medical Office - Endocrine Diabetes plays a vital role in delivering high-quality, patient-centered care within a specialty practice focused on diabetes and endocrine disorders. This position supports physicians and advanced practice providers through comprehensive clinical coordination, patient education, and care management. The ideal candidate is passionate about chronic disease management, thrives in a collaborative outpatient setting, and is committed to improving patient outcomes through education, advocacy, and evidence-based care.

    Key Responsibilities:
    • Provide direct nursing support to physicians and advanced practice providers in an Endocrine and Diabetes specialty clinic
    • Perform patient assessments, medication reconciliation, and clinical documentation in accordance with established protocols and standards
    • Deliver individualized patient education related to diabetes management, endocrine conditions, medications, and lifestyle modification
    • Coordinate care plans, referrals, diagnostic testing, and follow-up appointments to support continuity of care
    • Monitor and respond to patient inquiries, clinical messages, and test results in collaboration with the care team
    • Assist with population health initiatives, chronic disease management, and quality improvement activities
    • Ensure compliance with clinical guidelines, safety standards, and regulatory requirements
    • Collaborate with multidisciplinary team members to enhance patient engagement, access, and overall experience
    • Support clinic workflow efficiency, patient flow, and adherence to best practices in outpatient care
    Requirements:
    • Minimum Required Education: Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years experience. Preferred: Bachelor's degree in Nursing.
    • Required Licensure/Certification: State licensure as a Registered Nurse (RN). Basic Life Support (BLS) Healthcare Provider and any relevant certification as determined at position level.

      Minimum Experience: 6 months of nursing experience.
    • BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire.
