    Purpose:

    The Clinical Care Coordinator is responsible for the daily planning, organization, delegation, and completion of patient care.

    Responsibilities/Essential Functions (other duties as assigned):
    • Prioritizes, organizes, and individualizes multidisciplinary plan of care for treatment in a timely manner.
    • Acts as a liaison with the verification and authorization departments.
    • Ensures patient education is conducted and documented with patients and families.
    • Seeks consultation from appropriate resources/ departments as required to expedite treatment. Provides patient with any needed resources.
    • Acts as a liason and communicates and coordinates with patient caregiver, healthcare professionals both internal and external, and alternative patient settings as necessary.
    • Obtains medication insurance authorization from appropriate sources, in a timely manner.
    • Sends all prescriptions (premeds and injectable meds) to cCare pharmacy or coordinates with patients preferred pharmacy.
    • Schedules all cycles of infusions and related injections in the EMR.
    • Coordinates infusions with M.D./N.P. visits.
    • Effectively uses all resources necessary to provide the best possible care of patients and families.
    • Successfully completes other duties and projects as assigned.
    Experience, Qualifications/Education:
    • High School Diploma or equivalent required
    • Approximately five years clerical experience and knowledge of all aspects of clerical duties in the healthcare setting; experience with orientation and training Current CPR certification required
    • Knowledge of computer/telephony support, preferably in a health care environment
    • Strong customer service background, preferably in health care environment
    • Excellent verbal communication skills
    • Competence with computer processing functions and other standard office equipment
    • Ability to manage and prioritize multiple tasks
    • Ability to calmly and professionally resolve customer issues with diplomacy and tact
    • Ability to work independently with minimal supervision
    • Strong organizational skills
    Working Conditions:
    • This position functions primarily indoors in a medical / business environment. Employee may be subject to blood borne pathogens, radiation, bio waste and other medically-related factors, with exposure to moderate noise levels.
    • Visual acuity to read and compute screen and paper documents; close and distance vision, peripheral vision depth perception, ability to adjust focus; hearing acuity to converse with staff and customers; ability to walk, stoop, bend, kneel, and lift and move 25 pounds on a regular basis; ability to sit for hours at a time Employee will be exposed to moderate noise levels, interruptions, and occasionally asked to work evening and weekends as needed.
