Job description
Community Health is a primary care network offering comprehensive programs in wellness, dental, behavioral health, and pediatric specialties across eight locations in Vermont.
We are seeking a Master's Level Licensed Counselor to provide mental health treatment to children and adults in the Southern-Central Vermont area.
Conduct individual, family, and group therapy using evidence-based techniques.
Collaborate with multidisciplinary teams to develop personalized treatment plans.
Maintain accurate client records and facilitate referrals.
Communicate effectively within healthcare teams and participate in patient care planning.
Administer assessments and screenings.
Qualifications
Master's Degree in Social Work, Psychology, or Human Services field.
Licensed clinician (LMHC, LICSW, LMFT, LADC).
Empathetic, client-centered, and proficient in EMRs.
Strong interpersonal and communication skills.
Commitment to ethical standards and client confidentiality.
