Job Description
We are seeking a qualified Acute Care Hospital Case Manager to perform care coordination and discharge planning in an acute care facility. The ideal candidate will have a minimum of 2 years of hospital case management experience and will be responsible for ensuring efficient patient care from admission to discharge.
Responsibilities
- Conduct comprehensive assessments of patients to develop individualized care plans.
- Coordinate care across multiple disciplines to facilitate efficient patient transitions.
- Utilize InterQual and DRG criteria to manage patient cases effectively.
- Participate in family meetings and assist with the development of treatment plans.
- Arrange post-discharge services, including DME and SNF placements.
Required Experience / Certifications / Licensure
- Minimum of 2 years of acute care hospital case management experience.
- Strong knowledge of care coordination and discharge planning processes.
- California state nursing license required; pending licenses accepted if active upon start.
- Familiarity with Epic charting system is preferred.
